The Health Professions Student Loan is often referred to as a Pharmacy Loan at 美高梅官网 since Pharmacy students are the only ones eligible to receive it.聽

Read, complete and submit the appropriate paperwork for your聽Health Professions Student聽Loan Master Promissory Note (MPN) to be valid聽补苍诲听your accepted loan funds disbursed to your bill.

It may take up to聽3 days after completing the promissory note that all systems will be updated. Once the updates have occurred, your loan will be ready for disbursement based on the terms of your loan.

As with all loans, a borrower needs to submit paperwork that is the legal acceptance of the loan, shows an understanding of the process, and provides contact information upon repayment.

  • Included in the request is information on 2 references. These references are not responsible for paying back the loan. They will only be contacted should the 美高梅官网, the U.S. Department of Education or their agents need to locate you.
  • All areas must be completed. You will be contacted and your loan delayed without a complete form.

  • Print and complete from the hyperlink above.
  • On first page, fill in your name and the聽美高梅官网聽颈苍听Cincinnati, OH聽as appropriate.
  • Skip the "Schedule of Advances" table as 美高梅官网 uses this note as a Master Promissory Note (MPN). You will receive notification of disbursement via email each time a new loan is secured under the terms of this loan.
  • After reading the terms聽of the loan, sign as indicated.
  • On page 4, check and initial for this loan to be used as a Master Promissory Note (MPN) for future聽borrowing in this loan program.聽Otherwise, you will be required to聽complete this requirement each聽year you are given a loan.
  • The area for entrance and exit on Page 4 can be skipped.

Documents must be completely filled out and submitted at the locations below. Only after verification that all information is complete will you be sent a disclosure statement and your loan disbursed to your聽聽once we are within 10 days of the term and if you are registered for classes at least half-time.
