Important: Applications are only reviewed for current鈥搕erm eligibility. It is the student's responsibility to submit an application by the application deadline listed above.聽The application deadlines are strictly enforced, and submissions for past鈥搕erm eligibility will not be considered.

Also聽note: Fee payment deadlines cannot be extended while a student鈥檚 residency application is being reviewed. Students may opt to pay by using the University鈥檚 tuition payment plan. Information on聽payment plans聽can be found on the Bursar's webpage.

Required Supporting Documentation

C1: Financially Dependent Upon an Ohio Resident

Copy of BOTH student鈥檚 and parent/spouse鈥檚 Ohio driver鈥檚 license/state ID
Copy of marriage certificate (if residency is based on spouse)
Copy of parent/spouse鈥檚 federal & state tax returns for the most recent tax year
Current and past leases or deeds showing at least 12 consecutive months of Ohio domicile
Current Ohio vehicle registration
Copies of current immigration or Visa status for BOTH student and parent/spouse (if applicable)

C2: Financially Independent and Living in Ohio

Personal Income Declaration
Copy of student鈥檚 Ohio driver鈥檚 license/state ID
Copy of student鈥檚 federal & state tax returns for the most recent tax year
Current (and past) leases or deeds showing at least 12 consecutive months of Ohio domicile
Proof of financial resources for the past 12 months (e.g. paystubs showing Ohio tax withheld, unemployment benefits, state benefits, investments, financial aid, bank statements, etc.)
Current Ohio vehicle registration
Ohio voter registration
Copy of current immigration or Visa status (if applicable)

C3: Financially Dependent Upon a Parent/Spouse With Ohio Employment (Immediate Residency)

Copy of BOTH student鈥檚 and parent/spouse鈥檚 Ohio driver鈥檚 license/state ID
Letter from the student鈥檚 parent/spouse鈥檚 employer, printed on that organization鈥檚 letterhead stationery, affirming their full鈥搕ime employment and specifying of transfer or hiring
Copy of parent/spouse鈥檚 pay stub showing year鈥搕o鈥揹ate earnings and Ohio tax withholding
Copy of marriage certificate (if residency is based on spouse)
Copy of parent/spouse鈥檚 federal & state tax returns for the most recent tax year
Current lease or deed establishing that the student and parent/spouse are residing in Ohio
Current Ohio vehicle registration
Copies of current immigration or Visa status for BOTH student and parent/s

C4: A Veteran, a Spouse of a Veteran, or Dependent Upon a Veteran (Ohio G.I. Promise)

Copy of the 鈥淐ertificate of Release or Discharge from Active Duty鈥� (i.e. DD Form 214, member 4) issued by聽the US Department of Defense
Current lease or deed establishing the student resides in Ohio (veteran and dependent must both live in Ohio)
If the student is a veteran鈥檚 dependent, a copy of the veteran claimant鈥檚 most recent federal income tax return (page 1 & 2); Copy of marriage certificate (if residency is based on spouse)

C5: A Veteran Who Is the Recipient of Federal Veterans鈥� Benefits Under the 鈥淎ll-Volunteer Force Educational Assistance Program,鈥� 鈥淧ost-9/11 Veterans Educational Assistance Program,鈥� or Any Successor Program

Current lease or deed establishing that the student is residing in Ohio
Copy of the letter/certificate of eligibility for GI Bill benefits

C6: A Recipient of the Federal 鈥淢arine Gunnery Sergeant John David Fry鈥� Scholarship or Transferred Federal Veterans鈥� Benefits Under Any of the Programs Under the C5 Rule

Current lease or deed establishing that the student is residing in Ohio
Copy of the letter/certificate of eligibility for GI Bill benefits

C7: A Veteran Who is a Recipient of Federal Veterans鈥� Educational Assistance under the 鈥淰eterans Readiness and Employment鈥� rule or the 鈥淪urvivors鈥� and Dependents鈥� Educational Assistance鈥� rule (effective Summer 2022)

Current lease or deed establishing that the student is residing in Ohio
Copy of Tungsten-Network VR&E Enrollment Authorization Purchase Order聽or the letter/certificate of eligibility for GI Bill benefits

C8: A Graduate of an Ohio High School and Living in Ohio (Forever Buckeye)

Current lease or deed establishing that the student is residing in Ohio
Copy of student鈥檚 Ohio high school transcript or verification of student鈥檚 graduation on the high schools鈥� letterhead stationary (a copy of a high school diploma is 苍辞迟听sufficient evidence of high school graduation)
Copy of current immigration or Visa status (if applicable)

C9: A Recipient of a High School Equivalency Certificate (GED) in Ohio, and Living in Ohio (Forever Buckeye)

Current lease or deed establishing that the student is residing in Ohio
Copy of student鈥檚 certificate of high school equivalence
Copy of current immigration or Visa status (if applicable)

C10: Active Duty Military Personnel Whose Spouse or Dependent Lives in Ohio

Copy of spouse/parent鈥檚 Leave and Earnings Statement
Copy of marriage certificate (if residency is based on spouse)
Copy of parent鈥檚 federal tax returns (page 1 & 2) for the most recent tax year (if residency is based on parent)
Current lease or deed establishing that the student is residing in Ohio

C11: A person pursuing a graduate degree, graduate certificate, and/or a professional degree program, who has previously earned a baccalaureate degree from an Ohio institution of higher learning (Effective Fall 2020)

Current lease or deed establishing that the student is residing in Ohio
Copy of official transcript鈥攐r鈥攙erification of degree on school letterhead
Copy of current immigration or Visa status (if applicable)

E1: Financially Independent, Employed on a Self-Sustaining Basis, and Attending Part-Time (Undergraduate: 11 Credit Hours or Fewer; Graduate: 9 Credit Hours or Fewer) (Conditional Residency)

Personal Income Declaration
Copy of student鈥檚 Ohio driver鈥檚 license/state ID
Letter from the student鈥檚 employer, printed on that organization鈥檚 letterhead stationery, affirming their employment and specifying student鈥檚 date of hire, current salary, and the minimum number of hours they are required to work each week
Copy of student鈥檚 pay stub showing year-to-date earnings and Ohio tax withholding
Current lease or deed establishing that the student is residing in Ohio
Copy of current immigration or Visa status (if applicable)

E2: Active-Duty Military (Or Dependent/Spouse) and an Ohio Resident, but Stationed Outside of Ohio

Copy of student鈥檚 or parent鈥檚 Leave and Earnings Statement (LES) or Ohio income tax return
Copy of marriage certificate (if residency is based on spouse)
Copy of parent鈥檚 federal tax returns (page 1 & 2) for the most recent year (if residency is based on parent)

E3: Active-Duty Military (Or Dependent/Spouse) From Out-Of-State and Stationed in Ohio

Copy of service member鈥檚 current military orders for Ohio
Current lease or deed establishing that the student is residing in Ohio
Copy of marriage certificate (if residency is based on spouse)
Copy of parent鈥檚 federal tax returns (page 1 & 2) for the most recent tax year (if residency is based on parent)

E4: An Ohio Resident Transferred Outside of the US by Their Employer or Their Spouse/Parent鈥檚 Employer

Letter from employer, printed on that organization鈥檚 letterhead stationary, affirming that the employee was an Ohio resident at the time of the transfer, date employee was transferred outside of the United States, anticipated date of return to the United States/duration of assignment, and employer contact information
Copy of employee鈥檚 federal & state tax returns for the most recent year
Copy of marriage certificate (if residency is based on spouse)

E5: A Migrant Worker in Ohio (Or Dependent Upon)

Letter from the Ohio Department of Job and Family Services verifying that the migrant worker has been working in Ohio at least four months during each of the previous three years
Copy of employee鈥檚 federal & state tax returns for the most recent year
Copy of marriage certificate (if residency is based on spouse)

E6: In a Community Service Position (Or Spouse/Dependent Upon)

Copy of student鈥檚 Ohio driver鈥檚 license/state ID
Copy of tax form for the year prior to entering the community service organization and a letter from the community service organization verifying the start and end dates in the program (VISTA, AmeriCorps, City Year, Peace Corps, Teach for America)
Copy of employee鈥檚 federal & state tax returns for the most recent tax year
Copy of marriage certificate (if residency is based on spouse)

E7: Returning to Ohio Due to Marital Hardship

Copy of court papers verifying that legal action to end the marriage has been taken
Notarized letter聽that includes the following information:聽dates you or your parent/legal guardian has lived in Ohio, and the years you or your parent/legal guardian has paid taxes in Ohio
Current and past leases or deeds showing at least 12 consecutive months of Ohio domicile for the qualifying parent/guardian
Copy of parent's/legal guardian's Ohio personal income tax return (pages 1 and 2) for the most recent tax year

E8: A Member of the Ohio National Guard

Copy of the enlistment papers or an official letter from the personnel officer verifying enlistment
Copy of marriage certificate (if residency is based on spouse)
Copy of parent鈥檚 federal tax returns (page 1 & 2) for the most recent tax year (if residency is based on parent)

Personal Income Declaration Requirements

If you are submitting an Ohio Residency Application and requesting residency under a C2 or E1 classification, you will be asked to complete a聽Personal Income Declaration. You will need to calculate and provide 12鈥搈onth totals of your support and expenditures. You may want to determine these amounts in advance of starting the application.
